Primary syphilitic ulcers (or chancre) on the penis, vulva, or rectum tend to be single and are not recurrent. When they occur, they are usually indurated with a raised swollen border. They have a red smooth base and may produce a serous exudate. In addition, chancres are not typically painful, although they may be tender when manipulated. Primary syphilis resolves spontaneously but only after 2-3 weeks, not 7-10 days as in this case. The classic appearance of a primary syphilitic ulcer is shown in the photograph below:

Notice the edges of the lesion and the red shiny base. Notice also that there is only a single large ulcer, which is typical for syphilis.

Secondary syphilis may also produce genital lesions also; however, these are also not recurrent or painful as in this husband’s case, and they appear more plaque-like than ulcerated. Try a more likely option.
